Low Testosterone In Women: Causes, Symptoms & Treatment
The rate of new diagnosed cases of type II diabetes in 10–19 year olds rose most sharply in Native Americans (8.9%) an, Asian Americans/Pacific Islanders (8.5%) and non-Hispanic blacks (6.3%) (23). Your ovaries, adrenal glands and other tissues and cells produce testosterone. Too little or too much testosterone can affect your overall health and impact the levels of other sex hormones in your body. The side effects of testosterone on females aren’t fully understood. Many treatment methods for low testosterone are unregulated and not widely studied. Only you and your healthcare provider can decide what’s best for you based on your symptoms and bloodwork.
In men, testosterone levels increase from puberty to adulthood and then progressively decline starting by the fourth or fifth decade of life (4). One small study investigated this question by looking at groups of men across different age groups who were in “very good or excellent health” (5). The authors found no statistically significant difference in serum total testosterone levels across the cohorts grouped by decades of age. Their data support the idea that “the decline in serum T with male ageing is a non-specific effect of the common co-morbidities that accumulate during ageing” (5). Because of circadian variations in testosterone levels, serum testosterone measurement should occur in the morning, or within two hours of awakening in shift workers (Figure 19 ).
Primary hypogonadism happens when something is wrong with your testicles that doesn’t allow them to make normal levels of testosterone. Male hypogonadism is a medical condition that can affect people with testicles at any age from birth through adulthood. Your hypothalamus and pituitary gland normally control the amount of testosterone your testicles produce and release. Your body usually tightly controls the levels of testosterone in your blood. Levels are typically highest in the morning and decline through the day. Testosterone therapy is not recommended for those who have experienced or are at a high risk of prostate cancer.
Those low levels cause decreased testosterone and sperm production. All men with low testosterone levels are at an increased risk for developing osteoporosis, or weakening of the bones, and should receive counseling on osteoporosis prevention. Though it is not included in the ASA guidelines, I recommend to consider obtaining a DEXA scan (which evaluates bone density) in men with a baseline total testosterone of less than 200 ng/dL. See the “Osteoporosis” section of this website for more information. For example, let’s say a man who has been taking exogenous androgens for low T comes to my office for fertility treatment.

Hematocrit is a measure of the percentage of red blood cells on the blood. As mentioned earlier, polycythemia— excess amounts of red blood cells—is a possible side effect of TRT.
You can get it in different ways, such as in a shot, through a patch or gel on the skin, or in a tablet you place between your cheek and gum. A blood test is usually done to find out if you have low testosterone. If your doctor thinks low testosterone could be related to another medical problem, other tests may be done. If a man has shown some breast development, doctors may check his levels of estradiol, a form of estrogen.
For men using gels, serum testosterone should be measured at least 4 hours after the last application. Other things being equal (haematocrit, bone density, and symptoms), treatment aims to achieve testosterone levels in the reference range (mid-range for gel; low end at trough for injectables). Decision to continue testosterone therapy requires ongoing holistic review of the original indications for initiating it — along with intended benefits of continuing — versus the potential risks of therapy. However, consistent hormone replacement therapy helps improve sex drive, ease symptoms of depression and increase energy levels for those experiencing low testosterone. This is known as hypogonadotropic hypogonadism because there are low levels of luteinizing hormone (LH) and follicle-stimulating hormone (FSH).
